What If I Lose My COVID-19 Vaccination Card?

Joseph said his wallet was stolen, and he wonders how he can get a new COVID-19 vaccination card. 

The first step to getting a new vaccination card is to contact your vaccine provider, according to public health officials. The Ohio Department of Health recommends contacting your local health department if you can’t contact the vaccination site.

Summit County Public Health has a form on its website to request a new card, but the agency can only track down vaccinations that happened in Ohio, said spokesperson Marlene Martin.

If you were vaccinated in another state, you have to contact a health department there, Martin said.

The Ohio Department of Health (ODH) recommends taking a picture of the card on your phone in case you lose the physical copy. ODH also advises against laminating the card, in case booster shots are needed in the future.
